The fourth and latest installment, Trauma Center: Under the Knife 2, was released for the Nintendo DS on July 1, 2008 in North America, and August 7, 2008 in Japan. The game's story takes place in 2021 and features Derek Stiles and Angie Thompson as main characters once again; some other characters from the original game also returned, and new ones were introduced as well, like Adel Tulba. While some features of Second Opinion, such as the new tools, are carried over into this game, the new plot elements and characters from that game are not.

The story is set in the year 2018, where diseases that were previously thought to be incurable are nearly things of the past. The player assumes the role of Derek Stiles, a doctor at Hope Hospital. After the near-death of a patient due to his negligence, he encounters the scene of a car crash and ends up working on a patient with incredibly severe injuries to the heart. Just as it seems like the patient is done for, a power within Derek is unlocked, and miraculously saves the patient. The power emerges once more, and Derek saves another patient when a seemingly routine operation gets dangerously out of hand. His superiors recognize the ability. They tell him that his power is said to be that of a descendant of the Greek god of medicine, Asclepius, and his gift is known by the same name, though is more frequently referred to as the Healing Touch. He practices his use of the ability, though is left weakened after forcing the power to activate under his own will. After performing another operation, Derek blacks out for a short period of time. Dr. Kasal advises Derek to avoid using the ability due to the toll it takes on his body, and Director Hoffman tells him to outright turn his back on the ability, saying that the responsibility is too great for him to handle. After saving Linda Reid, a teen with a strange parasite (eventually known as Kyriaki), he learns of a new man made disease known as GUILT, which is distributed by a medical-terrorist organization known as Delphi. It is at this point that Derek decides to embrace his ability and use it to end the threat of GUILT. He then begins working for Caduceus USA as their main surgeon, where he heals a young girl who has the second known strain of GUILT (Deftera). During a conference, Delphi sets a bomb with the intent of killing the doctors attending. However, with the help of Cybil Myers, he successfully defuses the bomb. After this, he flies to Africa to aid Caduceus Europe, and encounters the third strain of GUILT (Triti) in a young boy. Back in America, Derek encounters the next strain of GUILT (Tetarti), which is only manageable after finding samples and using them to produce a few serums that are used to counteract the GUILT. Greg Kasal ends up being among those infected with the new strain, and is saved by Derek. Afterwards, Richard Anderson, the Head of Caduceus USA, becomes infected with a strain of GUILT (Pempti) that seems to be resistant everything that is thrown at it. Eventually, Victor Niguel, Caduceus USA's head researcher, manages to come up with measures to defeat the GUILT and after much resistance, the strain is finally removed from Anderson's body. However, Anderson succumbs to the toll taken on his body and dies soon afterwards, leaving Hoffman as the new director of Caduceus USA. Not long after this, outbreaks of Kyriaki, Triti, and Tetarti emerge and are thwarted by Derek and the other surgeons. Then, one night, a man breaks into Caduceus USA and ends up infecting Cybil with the sixth strain of GUILT (Paraskevi), though Derek manages to remove the parasite from Cybil's body. The man who broke in turned out to be Kenneth Blackwell, the father of Derek's assistant Angie, and they manage to trail him back to a research lab. There, they encounter the final strain of GUILT (Savato), first an immature form within one survivor, and then the main form within Blackwell himself. Practically being death incarnate, Savato proves to be a difficult challenge for Derek, being so swift that not even his normal Healing Touch is able to let Derek deliver the final blow. Using the Healing Touch twice in succession, the Healing Touch intensifies to the point where time seems to stop, and Derek finally manages to destroy Savato. Blackwell offers his full cooperation for saving his life and rejects Caduceus's offer of amnesty, wishing to serve out his term in prison to atone for his actions. Blackwell's information leads to a great wealth of knowledge about Delphi's operation and eventually the location of Delphi's headquarters. When Derek and Angie assist with a raid in cooperation with Caduceus Europe, they find that Delphi has been using children to incubate the seven strains of GUILT, and after curing the patients, Derek and Angie encounter Delphi's leader, Adam. What they also find is that Adam is practically a corpse, heavily infected with GUILT himself. They operate on all 7 strains and rid the world of GUILT. With the threat of GUILT eliminated, Derek becomes well known as the man who saved the world.

The story is set in the year 2021, three years after the threat of GUILT had been eradicated and seven years prior the events of Trauma Center: New Blood. The Republic of Costigar in Africa is a country that has been plagued by warfare for over a decade, and is finally on the road to peace. However, they still struggle with attacks by guerrilla militants, land mines, and many refugees. Despite this, the country is slowly on its way to becoming a peaceful nation. Countries around the world have been sending aid to the country, as well as building hospitals to help the people. However, they are very short on staff and need more to be able to take care of all of the patients. Doctor Derek Stiles, along with his faithful nurse Angie Thompson, travel to Africa to one of the refugee camps called Zakara. When the game begins, they have been working there for several months, both to help patients and investigate a new fever. A new doctor, Adel Tulba, arrives and is taken under Derek's wing. Derek eventually operates on a child soldier suffering from the fever they had been researching, and uses the Healing Touch for the first time in months, astonishing Adel. Soon after, Derek and Angie are called back to Caduceus, and Derek asks Adel to accompany them as part of a program to train doctors more effectively. At Caduceus, Adel settles in and begins training under Tyler Chase, while Derek and Angie investigate a new disease caused by GUILT, tentatively named PGS (Post-GUILT Syndrome.) The disease causes toxicosis in organs, as well as causing other effects such as tumors. Over the course of a few weeks, Derek and Angie operate on several PGS patients, including Linda Reid from the first game, aided by Victor Niguel, who has created a sedative to fight PGS. Derek and Angie eventually meet Emilio Juarez, one of the "Sinners" - children kidnapped by Delphi in the original game, and his nurse, Heather Ross. Emilio is treated for PGS and is given a liver transplant, but a new form of GUILT emerges from the transplant liver, much to the shock of Caduceus. Emilio and Heather return to a private institution following the operation. Soon afterwards, Caduceus is approached by Reina Mayuzumi, the president of a large pharmaceuticals company, Acropolis, to manufacture a medicine for PGS victims. Mayuzumi attempts to seduce Derek and offers him a lucrative position at Acropolis, which he refuses. She has him perform an operation while being recorded, in which Derek is forced to use his Healing Touch. Soon afterwards, a GUILT outbreak occurs at the institution Heather and Emilio are at, and Derek and Angie rush to the scene. Heather and Emilio are among the infected, and Emilio insists Derek operate on Heather before him, as he has GUILT antibodies. Derek successfully operates on Heather and two others, but Emilio dies before Derek can operate on him. Soon afterwards, Angie angrily blames him for Emilio's death, saying he should have relied less on his Healing Touch. Several days later, Adel suddenly collapses and is found to be suffering from the same fever as the child in Africa. Derek attempts to operate on him, but finds he cannot use his Healing Touch. Dr. Robert Hoffman, the head of Caduceus, takes over, and Derek soon realizes he has completely lost the Healing Touch. He goes into a slump and is transferred back to Hope Hospital, where he is mentored by Drs. Greg and Cybil Kasal, from the original game, as well as nurse Mary Fulton, who acted as Derek's assistant in the opening of the first game. After several weeks of working there, including a night wherein he is forced to operate on several emergency patients, Angie visits him and the two go to visit Angie's father in prison, Kenneth Blackwell. Blackwell collapses while they are there and is found to be suffering from mutated tumors on his heart due to PGS. Though Derek operates on him, one tumor appears that cannot be treated without the Healing Touch, and he gives up in frustration. Angie desperately pleas for Derek's assistance, and he is reminded of his own father's death. He successfully invokes the Healing Touch, saves Blackwell, and is transferred back to Caduceus. Some time later, Dr. Hoffman is approached by Patrick Mercer, the head of a new medical organization, the Hands of Asclepius, which works to train doctors. Adel quits Caduceus and joins the HOA, furious that Derek 'refused' to use the Healing Touch to save him. Soon after, a minor GUILT outbreak occurs at Acropolis, and Derek and Angie discover to their astonishment that Adel is able to use the Healing Touch on the patients there, though he is much more detached in terms of personality. Mercer later explains that he is training doctors to use the Healing Touch by using the recording of Derek's operation, provided by Mayuzumi. Derek and Angie return to Acropolis, but Mayuzumi is kidnapped by Delphi, the terrorist organization responsible for GUILT, while there. The police, HOA, and Caduceus eventually lead a raid on Delphi's headquarters, where Mayuzumi is found and operated on after being found to harbor the Pempti strain of GUILT. During this period, Blackwell is released and begins working for the HOA, and Heather comes to work for Caduceus. Delphi continues to cause GUILT outbreaks, and while assisting the police on a raid on a small Delphi hideout, Derek and Angie are kidnapped and taken to Delphi's main HQ, where they meet Heinrich von Raitenau, grandson of Adam from the original game and the new head of Delphi. von Raitenau threatens to use Angie as an incubator for new GUILT, but Derek is able to use his operating tools to break open their cell lock and call the police. In the ensuing raid, Caduceus is forced to stand by and the HOA take the lead. Derek, Angie, and Adel eventually corner von Raitenau in the basement, and Adel injects von Raitenau and his children - who he had been using as incubators for GUILT - with a new antibiotic. The antibiotic triggers the GUILT, strengthening it and sending von Raitenau and his children into critical condition. Before they can operate, Adel collapses, and Derek is forced to perform all three operations alone. von Raitenau is eventually taken to prison. Several weeks after that, Derek and Angie operate on a major tennis player in secret, discovering she is harboring a new strain of GUILT. With some prompting, she admits that she took special steroids provided by the HOA and Acropolis, and that they greatly enhanced her concentration and physical prowess. Meanwhile, Heather visits a hospital where her mother is in a coma. She has an icy conversation with her father, Patrick Mercer. The police and Caduceus raid the HOA and Acropolis headquarters acting on info provided from Blackwell, who had actually begun working for HOA as a means of infiltration and had been reporting their activities back to Dr. Hoffman. Mayuzumi and Mercer are nowhere to be found, but the doctors augmented by the HOA are suffering from debilitating aneurysms caused by GUILT. After operating on several patients, Derek, Angie, and Blackwell confront Adel, who is holed up in Mercer's office. Adel goes insane and tries to kill Derek using the Healing Touch, but is subdued and operated on, removing another new strain of GUILT. He returns to normal shortly thereafter. A few days later, Caduceus and the police raid Acropolis' main manufacturing plant, and Derek, Angie, and Heather confront Mercer, who reveals his true intent - he created new strains of GUILT in an attempt to reach a revitalizing stage that would revive his wife and Heather's mother, Tracy, who he removed from the hospital shortly after Heather's visit. He infects Tracy with a strain of GUILT, and she goes into a critical stage. Derek, Angie, and Heather operate on her, but Mercer escapes in the confusion. The three follow him into the basement of Acropolis, where they find Mercer, as well as Mayuzumi, who is hooked up to a series of tanks. Mayuzumi explains she has been keeping her youth and looks by voluntarily being the host of Aletheia, the final strain of GUILT. She taunts Derek, explaining that if she is forcibly removed from the machines or killed, they will release a gas of GUILT fatal to anyone who breathes it. Mercer injects her with a new compound in a final attempt to find a GUILT to save Tracy, and she is drained and reduced to an old woman. Tyler arrives with a new serum to treat it developed by Victor, and goes to disable the GUILT containers with Heather while Derek and Angie operate. The operation is successful, and the GUILT is neutralized. The police arrive and shoot Mercer, killing him instantly. Over the following few months, Caduceus regains its standing in the medical community, Adel returns to Africa, and Derek and Angie begin traveling the world to improve their skills. The game ends with them on a flight to Japan, reading a letter of thanks from Heather and discussing Derek's transfer, Angie choosing that moment to implicitly confess her feelings for Derek, with the closing text strongly suggesting a romantic relationship.

Up to Chapter 5 of Derek's missions, much of the story remains the same as the DS version, though the Wii version's Chapter 6 assumes the DS version's had already taken place. As Derek's story's progresses, Naomi's missions are unlocked. During her missions, it is revealed that she was working for Delphi, who knew of her ability and was using it for their own purposes. Naomi remains mostly unaware of the circumstances of Delphi until just before their raid, and also places their operation in danger at one point by insisting on operating on a reporter wounded after chasing down a car in which Naomi and a Delphi operative were passengers. Just before their headquarters are discovered by Caduceus, Naomi escapes with a GUILT sample which she plans to use as a bargaining chip. After Derek's Chapter 5 and Naomi's missions are cleared, Chapter 6 is unlocked. The plot of the chapter takes place at Caduceus Europe, where Derek and Angie have been invited to provide assistance and Dr. Hoffman has come to attend a conference held under near-total secrecy. There, Derek meets Naomi for the first time, who is now using her original name of "Dr. Kimishima" and granted amnesty in Europe, along with another doctor called Owen. Dr. Miller tells Derek about the Z-Cells, which are fruit of their research on GUILT in order to find a way to use it to help people. After Derek eliminates a somewhat stronger strain of Tetarti, his heart becomes infected with a combination of Kyriaki and Paraskevi, which leaves Naomi to heal Derek. Angie volunteered to assist Dr. Kimishima, but at certain moments was unable to see Derek in the condition he was in, crying many times and jumping to severe conclusions. Naomi succeeds, and time passes, during which, Angie described her true feelings to Derek. During the scheduled conference where Dr. Owen explains how a patient has recovered from a wound after a Z-Cell injection, Naomi expresses her doubts, claiming that GUILT was created to kill people, and probably could never be used to save them. Soon after the patient suffers from an injury that causes his blood to spray all over the conference room, infecting all the attendees with various strains of GUILT. After Derek and Naomi deal with the infected attendees, Owen tries to escape and reveals that he was working all this time to Delphi. Dr. Miller orders him to be stripped of his rank and taken away, and is then told that there is a problem in the lab where Adam is kept. Meanwhile, at the lab, Derek learns that the Z-Cells are taken from Adam himself, and that Hoffman has become infected with a strain of GUILT that is revealed to be a stronger version of Savato. In the final mission of the game, Derek and Naomi work together to take down the upgraded Savato and save Hoffman's life. Afterwards, Caduceus International proposes a treaty to deal with GUILT research which is quickly ratified. As for Derek and Angie, they are revealed to be assigned to northern Afghanistan, but not much else is revealed in the main game - though Tyler Chase, talking to Leslie Sears and Stephen Clarks, claims that both Derek and Angie deserve a long holiday because lately, they did not have much time to spend together alone... Naomi reveals that she cannot leave Europe without facing criminal charges for her work with Delphi, and parts ways with Derek.

Trauma Center: New Blood starts off seven years after the events on Trauma Center: Under the Knife 2. The game follows doctors Markus Vaughn and Valerie Blaylock, two surgeons with the "Healing Touch" ability who worked at the Concordia Medical Institute in Los Angeles. Dr. Vaughn moves over to Montgomery Memorial hospital in Alaska and is followed by Dr. Blaylock. They perform a few operations here, including one on Dr. Hoover, the director of Montgomery Memorial. In one episode at Montgomery Memorial, a man was shot with a rifle, and a young woman brings him to to Montgomery Memorial, and she actually turns out to be a nurse, who then helps them in the O.R. Later, they find out that this nurse is from Concordia and her name is Elena Salazar,and she had come to ask Dr. Vaughn to perform an operation to replace an implant in her pancreas. When the hospital closed down, Dr. Hoover tells them to return to Concordia. Markus is reluctant to return to Concordia, but realizes that he must go back. Once there, they learn from Professor Wilkens, an old acquaintance of Markus's, about a new and mysterious parasite called Stigma. After Markus and Valerie operated on Wilkens, who was infected with Stigma, the professor is kidnapped and all of his data regarding this new disease is stolen. Fearing for the worse, Dr. Vaughn and Dr. Blaylock are invited to join Caduceus, because of their valuable knowledge on Stigma. The first couple of cases were mostly kept from the public eye, but shortly after appearing on Miracle Surgery to take the show off the air, things only got worse. They were soon kidnapped by the Kidman Family, a criminal organization who have seen their skills and the profitability of Stigma, and forced to operate on patients intentionally infected with Stigma. Markus then confesses to Valerie and Elena that he was responsible for the creation of Stigma, which was from an accident from when he was working with Professor Wilkens. Professor Wilkens wanted to continue his research to the world with the intentions of becoming famous with the research. Markus, however, rejected the idea of using a potential threat to become famous and left for Alaska. After travelling to South America to treat more Stigma patients and to find Kidman and one of his companions (which ended in Kidman's death). When they returned to Caduceus, they discovered Cynthia Kazakov, a close friend of Valerie's, was in a relationship with a man named Master Vakhusti, who planned to give a similar implant to Elena that Cynthia has (The implant in question Cynthia has had a remote control which could stop the bloodflow, likewise, he planned to use it on Elena in order to coerce Markus in continuing Stigma research for him.). After removing the device from Cynthia, they encountered Vakhusti, who has the Cardia Stigma, Valerie and Markus deals with the final Stigma and finds out that Cardia was meant to be used as treatment for a terminal illness, but in doing so has made him sociopathic and insane. He dies acknowledging his sins shortly after the operation. As life returns to normal, Valerie, Markus and Elena contemplates on how it is up to them to save a life when needed.
